IPA and IDAS forms and templates

A suite of standard forms is available for making applications and amending approvals under IDAS.

The suite of forms consist of:

  • Form 1: Development application, made up of application parts A-R, assessment checklists, and attachments
  • Form 2: Request to change or modify an existing approval

These forms apply across the State regardless of the nature or location of the proposed development and regardless of the entity acting as assessment manager for the application.

Templates for the provision of owner's consent to the making of a development application are available. Owner's consent is not required for most IDAS development applications. Refer to IDAS Implementation Note No 4 for information on owner's consent and when it is required.

Hard copy forms

Hard copies of the IDAS forms are generally available from local governments, private building certifiers and IDAS state agencies. Private building certifiers and IDAS state agencies may only make available those parts of the form applying to their role in IDAS. Please note: hard copies of the forms are not available from the Department of Infrastructure and Planning.
